If there is no choice about using medication which contains alcohol, is it acceptable to use it and will we remain clean (pak) for the purposes of prayer, etc.

If there was no other choice except using alcohol, and it was necessary to use it for medical reasons, you can use it.

Following the school of Imam al-Shafi^y, alcohol is filthy, so you need to wash it before praying.

On the other hand, Iamam Rab^atur-Ra’yi, who was the sheikh of Imam Malik, he said that the alcohol is not filthy, he said that it’s haram to drink it and to buy it, but he said that it’s not filthy, so following what he said, you can pray while you have alcohol on your body or clothes without washing it.

So you can follow any one of these two scholars, but following Imam al-Shafi^y in this is better than following Imam Rabi^atur-Ra’yi.

I don't think these days there is ANY situation in which one would have no choice except to use medicine containing alcohol.
